Insulin autoimmune syndrome (IAS; Hirata syndrome) is a rare cause of hypoglycemia. It seems to be related to specific HLA class II alleles. Rarely, monoclonal antibody acts as an insulin-binding autoantibody: until now 10 cases have been described in association with a myeloproliferative disorder.
